Responsibilities may include:

•Performing comprehensive patient assessments.

•Providing patient visits as scheduled.

•Providing short term crisis intervention and individual or family counseling as needed.

•Utilizing our electronic medical record system, McKesson, to complete documentation.

•Serving as a patient and family unit advocate.

 


Requirements for this position include:

•Master's degree in Social Work.

•Social Work License or equivalent.

•Experience in Medical Social Work is preferred.

•Experience within the Hospice field is preferred.

•Valid driver's license, car insurance and reliable transportation.

•Willingness to travel throughout assigned service area.
