ViaQuest Home Health and Hospice is currently looking for a RN Case Manager to join our growing team. The RN Case Manager position is responsible as a member of the interdisciplinary team for the assessment and care of patients in their residences/homes. As appropriate, the RN Case Manager might interact with external sources to foster business-related relationships and increase business,
• Assist in staff development and site training of peers as needed
• Promote positive teamwork with other staff working in the home and the organization
• Assume primary responsibility for patient/family caseload including timely completion of paperwork
• Maintain open communication with attending physicians and other home health and hospice team
Candidate must hold at least an associate's degree or equivalent in Nursing from an accredited nursing program. The RN Case Manager must be licensed as a Registered Nurse in Ohio. Additional certifications/education may be required for specialty services in accordance with requirements of regulatory agencies. This position is required to maintain CEUs per state regulations. Minimum of two years of nursing experience is required, preferably in home health and the hospice field. Previous supervisory/management experience is preferred. Acceptable background check, valid Ohio driver's license, personal liability insurance, and driving record which allows candidates to be covered by agency insurance policy required.
The RN Case Manager must also possess all of the following: a sincere desire to provide service and support to persons in need; the ability to make sound judgments when given guidance and priorities; the ability to work as a team member to maintain consistency and quality in services; the ability to coordinate patient care services; and the ability to exchange support, constructive criticism and ideas with other staff members.
